Job Overview
We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our friendly and supportive clinical team at Oakview Medical Practice, located in the Vale Centre for Health & Care in Alexandria and at our satellite Practice, the Renton Integrated Healthy Living Centre. The ideal candidate will possess advanced clinical skills and a strong commitment to providing high-quality patient care. As an ANP, you will play a vital role in diagnosing and treating patients, promoting health education, and collaborating with other healthcare professionals to ensure comprehensive care.
Our team currently consists of 10 GP Partners, 1 salaried GP, 3 Practice Nurses, a Practice Pharmacist and an extensive administrative team. We also have working within the practice, 3 PCIP Pharmacist, 2 Phlebotomists, a Physiotherapist, a Community Link Worker, and we have access to a health board treatment room service in the building. We pride ourselves on providing high quality services, tailored to meet the needs of our 9,400 patients.
Duties
• Triage patients presenting to the practice and prioritise care.
• Conduct thorough patient assessments, including medical histories and physical examinations.
• Diagnose and manage acute and chronic illnesses, including sexual health, developing appropriate treatment plans.
• Apply clinical reasoning to manage undifferentiated and undiagnosed presentations.
• Prescribe medications and therapies in accordance with established protocols.
• Review and de-prescribe medications where appropriate.
• Order and interpret investigations such as blood tests.
• Provide health education and counselling to patients and their families on disease prevention and health maintenance.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Maintain accurate patient records and documentation in compliance with regulatory standards.
• Stay updated on the latest medical practices, guidelines, and advancements in healthcare.
• Participate in clinical supervision mentorship.
• Contribute to professional development activities within the team.
• Participate in audits and quality improvement projects.

• Ability to work independently as well as part of a multidisciplinary team.
