Garscadden Burn Medical Practice is seeking an experienced and motivated Clinical Pharmacist, to join our expanding multidisciplinary team within a progressive and forward-thinking General Practice, serving the Drumchapel community. The post will be based at Drumchapel Health Centre, Drumchapel, Glasgow.
This is an excellent opportunity for a clinically driven pharmacist, who is passionate about delivering high-quality patient care, leading Chronic Disease Management services, and playing a central role in modern primary care prescribing within Scotland.
The Role:
The successful candidate will lead and develop Chronic Disease Management (CDM) clinics across the practice, providing autonomous assessment, management, and prescribing for patients with long-term conditions within their scope of competence.
In addition to CDM responsibilities, the role will involve active participation in all aspects of primary care prescribing and medicines optimisation. The postholder will work closely with GPs, and the wider multidisciplinary team to support safe, effective, and evidence-based prescribing across the practice population.
This position would suit an experienced Independent Prescriber with a strong clinical background in either primary care or secondary care, and a proactive approach to patient-centred care.
Main Duties and Responsibilities:
• Lead and deliver Chronic Disease Management clinics
• Independently assess, treat, and prescribe for patients with long-term conditions
• Undertake structured medication reviews and polypharmacy reviews
• Support medicines optimisation and prescribing safety initiatives
• Participate in clinical audits, quality improvement projects, and prescribing governance
• Provide prescribing advice and support to the wider clinical team
• Support service development and innovation within primary care
Essential Requirements:
• Registered Pharmacist with the General Pharmaceutical Council (GPhC)
• Independent Prescriber qualification
• Experience delivering Chronic Disease Management clinics
• Strong clinical experience within either primary care or secondary care
• Excellent consultation, communication, and clinical decision-making skills
• Sound understanding of medicines optimisation and prescribing safety
• Ability to work effectively both independently and within a multidisciplinary team
Desirable Skills and Experience:
• Experience working within General Practice
• Knowledge of Scottish primary care prescribing systems and guidance
• Experience in polypharmacy and frailty management
• Familiarity with GP clinical systems
• Previous involvement in audit, quality improvement, or service development work
